目的 探讨甘露糖结合凝集素(Mannose-binding lectin, MBL)多态性与中国北方汉族人群HIV-1感染的关联。

方法 本课题采用病例对照研究,以91例HIV感染者(北京佑安医院)和91例健康人(北京同仁医院)作为研究对象,用焦磷酸测序技术(pyrosequencing)检测病例组和对照组MBL Exon ?第52、54、57三个密码子的点突变和启动子+4、-221、-550三个位点的多态性,用酶联免疫吸附法(ELISA)检测血浆MBL总浓度及血浆中有活性的MBL浓度,用流式细胞术检测HIV感染者的CD4+T淋巴细胞计数,用bDNA法检测HIV感染者的病毒载量。

结果 研究结果显示,等位基因B在HIV-1感染者中的分布频率显著高于健康对照组(0.18 vs 0.14)。HIV-1感染者的血浆MBL浓度(0.44)及血浆中活性MBL的浓度(0.61)比健康对照组的浓度要低。携带有A/B, A/C或者B/B基因型的HIV-1感染者与携带有A/A基因型的HIV-1感染者相比,血浆中MBL浓度、血浆中活性MBL浓度以及CD4+T细胞数量均偏低,但病毒载量偏高。

结论 MBL与中国汉族人群的HIV-1感染具有关联作用。携带有B等位基因型,低MBL浓度以及低MBL活性浓度的个体,更容易受到HIV-1病毒的感染。

Abstract:
 

Objective: Mannose-binding lectin (MBL) is a key molecule of the innate immune system and a component to bind carbohydrates of a variety of microorganisms, resulting in complement activation and opsonophagocytosis against various pathogens. However, there is no systemic investigation on the MBL’s role in innate immune responses against HIV-1 infection among Han Chinese.

Methods: A sample of 91 HIV-1 infected patients from Beijing Youan Hospital and 91 HIV-1 seronegative healthy individuals from Beijing TongRen Hospital Health Center were recruited. Six polymorphisms of MBL2 gene were genotyped by pyrosequencing. The quantitative measurement of serum MBL concentration and MBL complex activity were performed by ELISA. The CD4+ T-cell counts were determined by flow cytometry. The plasma viral loads of 91 HIV-1 infected patients were detected by bDNA method.

Results: The frequency of allele B in HIV-1 infected patients (0.18) was higher than in healthy controls (0.14). The median serum MBL concentration and median MBL complex activity in HIV-1 infected group was only 0.44 and 0.61 when compared to that in controls, respectively. The HIV-1 infected patients carrying A/B, A/C or B/B genotype in exon 1 had lower MBL concentration, lower MBL complex activity and lower CD4+ T-cell counts, but higher viral load when compared with those HIV-1 infected patients having A/A genotype.

Conclusions: Association between MBL and HIV-1 infection was detected among Han Chinese. The individuals with B variant, low serum MBL concentration and low MBL complex activity might have an increased chance of being infected by HIV-1.
